HowHowHow?

1) Sign the Consent Form
If you are 18-20 years old, take home the consent form (you can get a copy by emailing us at freeallergytest@gmail.com OR from the COM1 booth) and get your parent/guardian's signature.
If you are 21 and above, just come down and sign it yourself.

2) Register at COM1
Collect survey questionnaire and mouthwash tube.

3) Take the Skin-Prick Test and provide dermal patch sample. The result of the Skin-Prick Test will be known 20 minutes later.

4) Meanwhile, fill up the Questionnaire and follow instructions to perform your mouthwash.

5) 20 Minutes should be up. Let our trained professionals tell you your results.

6) Kindly submit your mouthwash samples and questionnaires to the Check-out counter. Our friendly receptionist will be consolidating and verifying the data.

7) Before you leave, remember to collect your $10 voucher^^.
